If you are interested in becoming a member of Paradigm Health, please complete the form below.  Our team will reach out quickly to set up your first consultation and answer any initial questions that you may have.

If you know the membership that you would like to select, we can get started right away with your comprehensive assessment.  If you would like to have a no-cost, no-obligation consultation with one of our physicians to discuss our offerings, our team will set that up as well.

No matter which plan you select, your comprehensive assessment will start with approximately 4 hours of testing at our practice. During that time we will obtain an ECG, perform extensive labwork, and you will undergo CPET, a DEXA scan, indirect calorimetry, and a movement, sleep, and nutrition assessment. After all results are returned (approximately 1 week), you will have a followup consultation with your physician to discuss your results and to create a Life Plan. This consultation will last 1-2 hours and there will be time to have all of your questions answered.
